Diabetes Healthcare Assistant

Castleman Healthcare Ltd, a federation of 12 GP Practices working across Poole North and East Dorset localities are looking for an experienced, versatile and highly skilled Healthcare Assistant with an interest or background in caring for Diabetic patients to work with our friendly and expert Diabetic Nurse Specialists to support the care of this cohort of patients.
As a Healthcare Assistant for our Diabetic Team you will have worked as a Senior HCA at a Band 3 level and have the Care Certificate or be willing to train for it.
**Job summary**:
To provide HCA services for diabetic patients across the 13 Castleman Practices, in a surgery setting, in their own homes and in care/nursing homes, supporting the Diabetic Nurse Specialists to deliver high quality care for our diabetic patients, their carers and their families.
**Job responsibilities**:
Work with set protocols to provide blood testing, blood pressures, flu vaccinations, B12s, dressings and other procedures as appropriate for a health care assistant to undertake.
Extended specialist skills: in house training to support patients with continuous blood glucose monitoring and insulin administration. Providing patient training and support. Providing training and support to practice teams.

- posting to external partners (e.g. LiveWell Dorset, Social Prescription team, carers lead, Healthy Choices etc.)
Be actively involved in the care of our Diabetic cohort of patients to provide:
- Yearly Diabetic Checks
- Education
- Foot checks
- Blood testing
- Blood pressure checks
- An understanding of QOF
- An understanding of the 9 Care Processes.
Ensure cleanliness and infection control procedures are adhered to at all times.
Ensure emergency equipment checks are carried out when required.

Administering B12, flu vaccinations, pneumonia vaccinations
Assisting with minor operations
Blood pressure recordings
Dressings
Ear Syringing
ECGs
Diabetic clinic support
Health promotion
Leg ulcer clinic support
Phlebotomy
Removal of sutures and staples
**Job Types**: Full-time, Permanent
