Care Coordinator- Full Time

Part of the job role also entails admin duties, such as: care plan reviews, team supervisions, completing risk assessments and rota management. Applicants must have a positive attitude and a willingness to learn and work in a fast-paced environment.
Full time hours 37.5 hours per week, salary based contract so guaranteed hours, salary paid 5th every month.
This role is covering the areas Hayle - Redruth.
**Mileage and expenses paid fortnightly
**Responsibilities**:
- A positive, compassionate, solution focused approach to the delivery of a personalised care and support plan.
- A flexible approach to your hours of work.
- An ability to communicate with the people who use our services in a friendly, professional, and effective manner.
- An ability to communicate within a multidisciplinary team environment.
- The ability to remain calm and respond appropriately when faced with a crisis
- To monitor and continually review the health, well being and progress of the people who you care for and support, ensuring that any concerns, changes in the persons condition or environment are recorded and reported to your line manager at the earliest possible opportunity.
- To take part in on call system (no more than 7 days per 28 days). On call hours are between 0600-2200. On call is shared amongst management team and second on call is always available so you're well supported.
