Overview:
**Change Grow Live** are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.
As a **Recovery Coordinator** for Health In-reach you will provide a hospital-based service within an Integrated Drug and Alcohol Recovery Service to people with substance misuse needs.
The successful post holder will be based primarily in A&E, admissions wards and will provide rapid access to assessment, brief intervention and liaison, discharge planning and onward referral for service users attending hospital with substance misuse related issues. Along side this the Post holder will work actively with the GP surgeries across the East Sussex Drug and Alcohol service promoting the service and collating referrals.

**Salary**: £27,449.53 - £31,529.41

**Responsibilities**:
**About the role**:
- Work in close partnership with acute hospitals, GP services and the Change Grow Live community teams to develop and implement care plans for hospital attenders.
- To provide specialist assessment of patients attending/admitted to hospital with alcohol related issues using Alcohol Audit and SADQ screening tools.
- To provide brief interventions to patients scoring below 20 on the alcohol audit.
- Hold a small caseload of service users who are known to East Sussex Drug and Alcohol Service. The Hospital In-Reach Worker will provide drug and alcohol treatment, providing comprehensive assessments, coordinating the resettlement plan from hospital by liaising with relevant agencies.
- Build Links with local GP surgery’s and improve referrals into treatment.
- Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospital.
**About you**:
- Experience of working in Drug and Alcohol services and delivering psychosocial interventions
- Ability to manage challenging behavior in a calm and professional manner.
- Sound working knowledge of safeguarding.
- Sensitivity in working with complex patients.
- Experience of working positively with healthcare partnerships.
- An understanding of the processes of the local hospital trust and other healthcare providers.
