Service Coordinator

We have a fantastic opportunity within our continuously expanding organisation for an experienced Service Coordinator, with knowledge and understanding of adults with Learning Disabilities, Autism, mental health, forensic background and Challenging Behaviour.
**Skill set**:
Minimum of QCF/NVQ 3.
Must be a driver with daily access to car to fulfil role.
Excellent literacy skills.
Excellent communication skills.
Ability to work on own initiative.
Computer literate including basic knowledge of Microsoft Word.
A pro-active mindset, with a willingness to be flexible, agile & react positively to change.
Ability to develop strong working relationships with others.
Ability to work to Harmony care’s values.
Ability to participate in the on call rota.
**Experience of**:
Writing and reviewing person centred support plans of a high standard.
Writing and reviewing detailed risk assessments of a high standard.
Completing Person centred meetings with the service users and families of Harmony Care.
Overseeing the daily running's of 4 schemes, totalling 10 service users.
Managing the services medication.
Completing Medication and finance Audits.
Completing staff competency checks for meds, manual handling and dysphagia.
Leading staff, ensuring competencies are upto date, supervisions, spot check and yearly appraisals.
Your role is partially office based and partially out in the field managing your services.
