Leatherhead Primary Care Network have an exciting opportunity for a Care Co-ordinator to join our Team, supporting our 6 GP practices and residential homes across our community.

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring our patients receive the best possible care and service. Ideally you will have a good understanding of the NHS, but those with a transferable skill set are also encouraged to apply.

There are many projects running at anyone time and you could be deployed to support any of them including health & wellbeing, blodd pressure monitoring, diabetes support or pharmacy drug reviews. You should be happy to pitch in and support whatever area of the organisation may need your help at the time. You must be a confident communicator as you will have to liaise with both patients and health care professionals. You must also have strong IT skills, be self-motivated and be well organised.

We are able to accomodate school hours for this role, so it may suit someone looking to return to work after a period raising a family. We can also accomodate some home working however you would need to be in our local area, as face to face meetings will be required on a weekly basis (Cobham, Oxshott, Bookham, Leatherhead)

Part-time hours: 20-37 per week
