This new exciting post has been established to strengthen the link between our acute adult inpatient unit (WLH) and the Crisis Resolution and Home Treatment Team (CRHTT). This pilot post is an excellent opportunity for the post holder to shape and develop the role further by working closely with inpatient clinicians and multi-disciplinary team members to deliver a high quality, service user focussed service.

With a primary focus on treating people in the least restrictive environment, improving service user outcomes and reducing length of stay within WLH, the post holder will take the lead role in assessing the suitability of inpatients for supported leave and early discharge supported by the CRHTT’s. The post holder will be responsible for creating a comprehensive Home Treatment Plan in conjunction with the service user, carers, and community care team (if appropriate), ensuring this is shared with relevant parties. The post holder will be responsible for handing over the service user to the relevant locality CRHTT team, however responsibility of overall coordination of discharge will remain with the Inpatient Ward Team.
- The post holder will have responsibility for offering CRHTT advice, assessment or care as appropriate to any service users within Wotton Lawn Hospital.
- The post holder will support the wards to identify those suitable for early discharge/leave.
- To support the implementation of this new role and embed new ways of working which lead to improvements to service user care, reduce length of stay and improve access to acute hospital beds.

This post will be based within a Crisis Resolution and Home Treatment Team, however frequent attendance at Wotton Lawn Hospital will be required. This role does not currently require night or weekend working; however, weekend working may be considered in the future.
