Yorkshire Health Network is a federation of all 17 GP practices within the Harrogate and Rural area of the Humber and North Yorkshire ICB (integrated Care Board).

The Digital and Transformation Lead will manage and deliver a plan to build a robust digital infrastructure for the Knaresborough and Rural Primary Care Network through specific projects designed to improve patient care and access. These projects will drive improvement in integration and collaboration across the 6 Practices in the PCN.

NHS Pension scheme available and in recognition of the dynamic nature of this role, the hours and base of this post are flexible, with a requirement to independently travel to all of the PCN's Practice sites.

The Digital and Transformation Lead will:

- Support the PCN to identify and plan improvements.
- Deliver specific quality improvement projects and support staff at all levels in the PCN to take forward the change.
- Improve the use of existing and new technologies to deliver benefits to patients and staff.
- Support integration within the PCN and wider system.
- Use data to identify opportunities and to drive improvements in care quality and experience.

Project delivery will be underpinned by a quality improvement methodology, building staff skills and capability alongside enhancing patient access and experience.

The post holder will be expected to proactively project manage all aspects of the project life cycle, working closely with the relevant stakeholders to bring projects to fruition on time and within allocated budgets.

The responsibilities below outline the methodology and scope of the role in general terms.

Leadership and Management
- Develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools
- If required, procure and manage digital tools for the PCN
- Ensure all initiatives are supported by a clinical champion and practice managers
- Ensure all staff receive training on the digital tools within the PCN
- Remain up to date with the national and local PCN guidance
- Manage communication channels, ensuring buy in from all stakeholders
- Ensure inter-dependencies are identified and risk management registers are maintained
- Liaise with Practice Managers with regards to the impact of the changes as required and identified
- Feedback (as required) reports both written and verbal to the Clinical Director and stakeholders
- Ensure data quality is improved and data is utilised to understand demand, capacity, activity and improvements for patient access, improving operational efficiency.

Governance

Create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts and decision-making, which is accessible centrally

Communication & relationship building
- Work in partnership with the PCN leadership and management team
- Identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice
- Deliver technical information in a non-technical and engaging way to a range of stakeholders

Professional development
- Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

**Job Type**: Part-time
Part-time hours: 30 per week

**Salary**: £41,000.00-£43,000.00 per year
