We are looking for experienced Care Assistants or Support Workers on behalf of Yorkshire Community Healthcare, to support clients with complex health needs.

We are looking for both day and night support workers.

The role involves supporting a client both in their own home and out in the community, as required.

Working on a one to one or two to one basis, your role will include:

- working to a person centred support plan
- keeping accurate and up to date records and daily notes
- supporting client(s) with personal care and moving and handling
- liaising with other health professionals
- building strong and effective working relationships with clients and colleagues

We are looking for people who have:

- previous experience supporting clients with complex health needs, including Epilepsy and Mental Health
- sound knowledge and experience of supporting clients with moving and handling and personal care
- patience and empathy
- an ability to build positive relationships
- a commitment to providing the highest quality care and support
