**POST: Health Care Assistant**

**LOCATION: Lewis Square Practice**

**Reports to: Staff nurse/ Jennifer Turner Practice Manager**

**Responsible to: Lewis Square Partner GPs**

**JOB SUMMARY/MAIN PURPOST**:

- As an employee of ‘Lewis Square’ to assist in the deliver of a high quality of Treatment Room Services._

**Main duties/responsibilities**

Assist in delivering a high quality of care and attendance to patients.

To provide skilled nursing care, advice and support to patients/relatives within the Community. Assessing, planning, implementation and evaluating effective nursing interventions, in accordance with the ‘Lewis Square’ policies and Procedures and NMC Code Of Professional Conduct.

To liase with relevant staff within the practice in relation to information regarding patients.

To ensure safe handling, use, storage of equipment issued to patients.

Responsible for the Health & Safety of staff/patients/carers by adhering to all policies, procedures and guidelines and Health & Safety at Work Act in area such as:

- Manual Handling
- Clinical Waste
- C.O.S.H.H.
- Infection Control
- Child Protection
- Safe Environment
- Violence and Aggression against staff
- Vulnerable adults
- Fire Prevention

Responsibility to adhere to ‘Lewis Square Practice’ policies, procedures and guidelines re:
Reporting of accidents/incidents involving patient/carers/staff and investigation of same

Reporting of complaints

Reporting of sick leave/absenteeism

Maintaining confidentiality

Maintains accurate records with precise documentation and submits reports and statistical returns when required.

To accept, train and use all new computerised patient record systems in line with Treatment Room development.

Enter data onto the relevant patient information system either electronically or manually.

Obtain measurement of a patient’s weight and height, blood pressure, peak flow and oxygen saturation.

Perform urinalysis.

Support professionals with clinical activities.

Assist in the maintenance of stock levels.

Prepare and maintain medical equipment.

Provide assistance to nurses or GPs as required.

Act as a health educator, by advising and counselling in relation to promotion of health and prevention of disease.

To participate in screening procedures as required.

Responsible for safe use and storage of medication.

To participate in initiatives aimed at increasing community participation in health.

To communicate effectively within the team of community nurse, primary health care team, and with other professionals/agencies as necessary to maintain a co-ordinated approach to health care.

To provide relief duties to cover short term or emergency situations.

To participate in the setting and audit of standards to ensure the delivery of a quality service.

To participate in nursing and allied research as appropriate.

To accept responsibility for personal professional development.

- Maintain a clean and organized patient environment
- Document patient information accurately and timely

**Requirements**:

- High school diploma or equivalent
- Completion of a recognized healthcare assistant program or certification preferred
- Strong communication and interpersonal skills
- Ability to follow instructions and work effectively in a team environment
- Compassionate and caring attitude towards patients
- Basic knowledge of medical terminology and procedures
- Ability to maintain patient confidentiality at all times
- Physical stamina to perform tasks that may require lifti
