Our Children's community team are looking for a Health care support worker to join our small dynamic team.

The Children’s Community Support Worker will be responsible for providing a range of social and health care to children and young people with complex and acute health needs in their own homes and within the community setting.

The post holder will be expected to work with the families and wider MDT deliver high quality care. They will also be expected to work independently under the direction of their manager

Flexibility is required to ensure service needs are met as well as the ability to drive with access to own car

MAIN DUTIES/KEY RESULT AREAS
- has responsibility for care of equipment and resources used by self or others.
- contributes to the effectiveness of teams.
- will be expected to understand basic reflective techniques to enhance their self-awareness and to develop resilience when facing adverse situations. They must be able to report back on any difficult situations encountered to enable support and guided learning to be offered.
- will offer a range of care to children and young people in a variety of community settings.
- will follow plans of care and advanced care plans, undertaking defined procedures or therapeutic interventions, recording care given appropriately and reporting back progress or deterioration to the manager
- will exercise a degree of autonomy as they are working alone whilst recognising the limits of their competence and working to the policies and procedures of our organisation.
- must be skilled in communicating with children and young people and their families, acting as advocates, when necessary, within their delegated workload will be able to prioritise, plan and organise their work.
- must be able to work effectively in a team and participate in team development, design and development of service improvements and a range of quality assurance activities.
- must be able to deliver key health skills.
- Ability to interpret complex information to identify a family’s needs and to think creatively to identify appropriate support.
- Ability to negotiate and influence to bring about positive change for children, young people, and their families.

**Qualifications and/or experience**:

- A detailed understanding of systems, procedures, and policies relevant to Children’s Social Care e.g., child protection and safeguarding procedures, practice, and policy.
- Demonstrable experience of working with families, children, and young people in order to provide support, especially those with complex difficulties, including interpreting families’ needs.
- Ability to update knowledge of various acts and statutory guidance relating to children and young people.
- Knowledge of risk management and clinical governance.
- A sound knowledge and understanding of special educational needs and/or disabilities.
- Sound knowledge and understanding of person-centred approaches and the ability to ensure that these are used to empower children, young people, and their families.
- Understanding of the complexities of supporting children, young people, and their families.

Afternoons 15.00 - 18.00

**Job Type**: Part-time
