We currently have one permanent full time vacancy and 2.5 part time fixed term contracts until March 2025 available.

**Your next role**

You will be delivering a 1:1 or group based interventions. You will work closely with the Project and Team Lead, Programme Manager along with colleagues in the wider team and partners across the health and social care system and will focus on ensuring that the Live Well offer is evidence based, person centred, well planned, safe and effective.

You will find ways to reach out to those facing greatest inequality and are creative and collaborative in your approach, inspiring and motivating lasting change.
- You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”)._
- The ability to travel throughout the Borough will be required in order to be available to support sessions and events around the town._
