Tobacco Dependency Advisor

Smoking rates among people with a mental health condition are significantly higher than in the general population and there is a strong association between smoking and mental health conditions. The LPFT Tobacco Dependency Treatment Service (known as the QUIT team) aim is to offer a bespoke model of care designed to meet the needs of mental health service users to support patients to quit.
We recognise that tobacco dependency is a chronic, relapsing clinical condition that prematurely kills at least half of people who smoke. Smoking is seen as a medical condition that can be treated, rather than as a lifestyle choice.
We are looking for a motivated, enthusiastic individual who can work as an effective team member, demonstrate good personal communication skills and actively promote excellent team and interdisciplinary relationships.

To work with the tobacco dependency treatment service lead to provide behavioural support and treatment in relation to tobacco smoking.
To support the day to day running of the service to ensure safe, well organized and evidence based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of settings as required for the delivery of smoking cessation service.
To support the delivery of the service key performance and reporting indicators for tobacco dependency.
Organise own workload and work unsupervised in a busy team.
Maintain a high standard of work and prioritise own workload according to the needs of the service, individual needs including equality, diversity and social inclusion in all aspects of work.
Take referrals into the service and work with patients to identify the most appropriate tobacco dependency treatment pathway.
Identify and engage with individuals and groups from target areas.
Establish and maintain good relationships with patients accessing Mental Health Services and other members of staff, ensuring a high degree of confidentiality, complying with Information Governance policies at all times.
Support patients to make to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioural changes to improve their health.
Record and report activities and results.

To engage with inpatients and high risk outpatients to provide support and advice with regards to smoking cessation.
To offer advice on Nicotine Replacement Therapy (NRT) where appropriate.
To provide interventions to patients to improve their lifestyle and advise on further support available.
Administration duties in relation to recording patients' stop smoking attempts on LPFT clinical systems.
To undertake basic assessments and interventions related to smoking cessation
Contribute to and work as part of a team
Manage own workload efficiently under direction of the service clinical lead
Facilitate any transfer of care into Community Stop smoking provision, such as LA SSS or primary care including GP or Pharmacy
Signpost individuals to other appropriate services to further support their health and wellbeing needs.
Identify any risks relating to client care and escalate appropriately to senior members of the team for support as required
Offer therapeutic and supportive interventions to support a quit attempt
Take and record carbon monoxide readings
