**The role**

Do you have experience supporting vulnerable individuals going through the homeless pathway? BRC are currently looking for experienced Support Workers to help a leading complex needs support provider in central Plymouth to manage their caseload.

This service support clients that have experienced issues around housing, mental health and substance misuse to live more independently, this will include housing and tenancy related support.

**What they need**
- Processing referrals and undertaking triage assessments over the phone.
- Provide advice and signpost to relevant agencies.
- Provide tenancy related support
- Multi-agency working, responding to enquiries from professionals and partner agencies.
- To carry out needs and risk assessments enabling service users to reflect on their needs, and further aid their recovery + rehabilitation.
- Move on support
