Deliver person centred care putting the student at the heart of everything and helping them when needed to make their lives easier and improve their quality of life
- Take an active role in supporting students to develop their personal care and daily life skills at college and in the wider community in accordance with the agreed Education Heath Care Plan (EHCP) or equivalent outcome
- Active involvement, contribution and participation in team delivery and development of Independence provision for students to empower students to develop their quality of life
- Support students to attend external appointments accessing a variety of different transport methods
- Administration of student medication and support of student self-administration of medication in line with college policy and procedures
- Maintain accurate written and computerised documentation as defined by the Health & Social Care Act 2008 ensuring care plans and all associated student documentation and information is current and up to date in line with College audits and CQC requirements
- Take on the role of keyworker for identified students within the residence acting as a point of contact for parents/carers and carry out a monthly 1:1 catch up meeting with the student following up any outcomes from the meeting
- Provide pastoral support to all students, liaison with families, careers, support network and key support services across college to ensure relevant, effective communication
- Work flexibly across the college campus supporting students to access the residential and curriculum provision, providing bespoke support
- Timely and effective use of varying communication methods to contribute to student well-being, information dissemination and to develop partnership working within the organisation
- To work across all areas of the College supporting residencies as student needs dictate
- _T_reat everybody with dignity and respect at all times adhering to the Dignity in Care and CQC fundamental standard
- Encourage and promote positive social integration and inclusion within student peer groups where appropriate internally and externally
- Encourage and support students to use their time appropriately promoting a healthy lifestyle
- Contribute to creating an environment that focuses on the wellbeing and quality of life of the people we support
- Facilitate students wishes and interests during unstructured time both on and off campus
- Enhance the quality of life of our students preventing and reducing behaviours of concern by embedding the college Positive Behaviour Support (PBS) strategy
- Embrace change positively and adapt new ways of working where appropriate to continually improve and maintain the quality of the provision
- Role model best practice within the organisation

Short breaks 8.00-20.00 on a 2 week Rota

week1 Monday, Tuesday, Friday, Saturday Sunday

Week 2 Wednesday, Thursday

Hourly rate - £12.44 (new rate from april)
