We are looking for an individual to provide speech and language therapy at assessment and consultation at an advanced level of practise, who has good SLT knowledge and skills and is able to work with people who have challenging behaviours and be proactive in the development of the new service, as well as working collaboratively with other SLT colleagues within the Care Hub. The postholder will have access to professional supervision and support from AHP Leads working within Pennine Care. The postholder will have good leadership qualities and will support other SLT staff within the care hub as required.

The post holder will be an integral member of the learning disability multidisciplinary
team on the new Learning Disability Crisis Response unit. The unit is an admission
avoidance service for people with a learning disability with or without autism who are
experiencing a period of crisis. The unit will provide a trauma informed model of care, with least restrictive practice for people aged 16-25 years.

The postholder will develop and provide a highly specialist Speech and Language
Therapy service to service users who have complex communication and dysphagia
needs within the context of a multidisciplinary team. This will involve working with
vulnerable and extremely challenging individuals. Assessment and interventions will be delivered using person centred approaches within the unit and at times, across the learning disability care hub supporting other locality community learning disability teams.

The post holder will demonstrate leadership, will monitor and maintain professional
standards and will be involved in service development across the Learning Disability care hub including the supervision of staff. They will be expected to contribute to achieve the Trust and care hubs objectives in addition to making a positive contribution to the health of services users with learning disabilities.
The post holder will at all times act in accordance with the HCPC standards of conduct, performance and ethics.

To be responsible for the delivery of a Highly Specialist Speech and Language Therapy service to service users with a learning disability. This includes dealing with complex case management issues and generating appropriate strategies to manage these. Provide a high level of clinical and professional expertise in order to ensure the delivery of safe and effective multi-disciplinary pathways of care, in partnership with other members of the multi-disciplinary leadership teams. To be autonomous and responsible for the assessment, formulation and management of a specialist clinical caseload of which may have additional complex needs as a result of other diagnoses.

To make complex clinical decisions following assessment and develop highly specialist individualised care plans based on the evidence base and to develop appropriate strategies to facilitate communicative effectiveness. To act as a source of specialist information to other members of the team to ensure service users’ communication needs are recognised and met in line with the Accessible Information Standard. To demonstrate knowledge of and adhere to RCSLT Professional and Clinical and National and Local Clinical Guidelines, evidence-based practice and standards for advice and clinical interventions.
