We are looking for B6 registered staff who want to make a difference in helping people make the most of their lives and who also have a special interest in home treatment. The North West Home First Team provides a range of supportive interventions to prevent and manage crisis whilst supporting the individual's recovery in the community. They are the gatekeepers to inpatient admissions and support with the facilitation of early discharge. The team provide the service across the West Essex area and are based at the Derwent Centre in Harlow.

We are looking for a highly committed practitioner who is flexible, proactive, and can contribute to the good work, innovate new ideas and help drive service improvement whilst working collaboratively with a number of individuals and other partner organisations in order to an alternative to hospital admission for service users.

Applicants will be expected to have professional registration and relevant experience at B5 or above.

KEY RESPONSIBILITIES CLINICAL

The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programs of care for patients who are struggling with their mental health within their own home environment. There would also be the expectation for the post holder to facilitate gatekeeping assessments and support with early discharge.

The Home First Team provides intensive support for patients in the community, working closely with hospital, community services, primary care staff, social care professionals and other third sector organisations. They will be responsible for delivering treatment and helping to maintain standards of care within the Team. They will be expected to work collaboratively with individuals and their carer’s/family delivering person centered care. They will actively participate in the training and education of staff, families and carers as required. In addition, they will also be responsible for ensuring that an integrated service is provided and ensure that good multi-disciplinary working is maintained.
