Burton Addiction and O’Connor Centres is an award winning organisation offering support to individuals with drug and alcohol problems in a range of settings including detoxification and residential rehabilitation, recovery housing, social enterprises and community based therapeutic services.

This role is based in our Stoke Recovery Service a partnership between ourselves, North Staffs Combined Health Care Trust and We Are With You and this is an exciting opportunity to be part of this service within this partnership.

**Job Purpose**

We are looking for that special someone who is able to support our Service Users who have completed their clinical treatment and access SCDAS, ensuring they receive a coordination of interventions and activities to further their recovery which involves wider health and social care support.

Interventions in group and/or one to one settings are tailored to the Service User motivating them to achieve their person centred goals with a focus on achieving sustained recovery within the community

**Main Duties**
- Inform Service Users of their after care options and involve them in decisions and consent to motivate them to take opportunities to achieve sustained recovery.
- Undertake accurate and comprehensive assessments of need, identifying levels of risk and implementing ways of managing risks with clients
- Co -produce, monitor and review Service Users goals plans and motivate them to complete realistic actions to achieve long term recovery goals.
- Deliver activities and interventions which may be alongside peers and volunteer, in Stoke Recovery Service premises, partnership premises, community outreach locations and client accommodation.
- Identify and facilitate access to community resources to support recovery within the community.

**About You**

Ideally you will have:

- Sound working knowledge of the models of recovery within substance misuse services in practice
- Knowledge and understanding of harm reduction, and how this relates to substance misuse
- Understanding of the cause and effects of substance use on clients, families and the community.
- Experience of completing assessment of needs, support planning, implementing & evaluating holistic person-centred risk / aftercare plans to a high standard.
- Knowledge of the Policies and Procedures relating to Safeguarding of children and adults at risk and how this would be implemented within the workplace
