We’re looking for an enthusiastic Hospital-Based Youth Mentor to join our growing team at SMASH SMASH exists to empower young people to be their best self by increasing their confidence, improving relationships and increasing engagement in healthy and positive activities. We are looking for a self-motivated, enthusiastic and organised person who will take a leading role in the development and delivery of our hospital-based youth work and the mentoring of young people frequently presenting at hospital. **Job Purpose**

To place Youth Workers in the Emergency Department (ED) and in children and adult wards within Salisbury hospital, playing a crucial role in supporting Children and Young People (CYP) aged 11-25 years who present at ED, those admitted to the Children and Adult wards and those with Life Long Conditions, displaying low -medium level mental health issues/concerns/needs. The Youth Workers will adopt a holistic, compassionate and trauma informed approach, having the time and experience to begin to forge trusting relationships with CYP. The role will involve face to face and virtual support (using secure and safe virtual platforms) and will act as a link to longer term support available in the local community.

Your role will establish trust with young people who arrive at Salisbury Hospital. When in the hospital, there is an opportunity for the young person to reflect and be open to intervention. This moment may assist in redirecting the young person back into the community via a different pathway. Your role will build trust, support them through understanding positive change, and then work with them to build a referral to ongoing services in the community to connect them with a local support agency/charity that would best meet their needs, building resilience, and providing mentoring to prevent repeat occurrences that could lead to their hospitalisation. Once the Mentor has gained the young person's trust and agreement, they will prepare an assessment and referral form that includes the type of difficulties, services available for referral, and the type of communication the client prefers.

The role will include working at times when the hospital would value our presence, including regular evening and weekend sessions. The role will involve coordinating and undertaking all aspects of the service offered, including handling referrals, session preparation and delivery monitoring and evaluation. The post-holder will represent SMASH externally in the course of their work.

**Main Duties**
- To champion SMASH’s vision and values through facilitating excellent hospital-based mentoring and professional 1to1 mentoring sessions in the community, aimed at supporting young people to make healthy choices that reduce readmission.

- Work within a busy hospital departments including A&E, Medical Assessment Unit (MAU) and Childrens and Adult wards.
