- A Basic DBS is required

You will deliver an effective Meeting Centre programme for people living with mild to moderate dementia. To provide support to members and their family (carers) and to ensure the achievement of high professional standards of person-centred care in the accordance with Age UK Herefordshire and Worcestershire values.

The Dementia Services portfolio includes Dementia Wellbeing, Dementia Cafés, Maintenance Cognitive Stimulation Therapy and Dementia Meeting Centres across Worcestershire. The service aim is to support independence through the use of assistive technology, signposting to other support services both internally and externally and community assets unique to their own situation.

You will manage your work time effectively, have a passion for supporting older people and have the ability to be flexible in order to meet service needs.

**MAIN DUTIES AND RESPONSIBILITIES**:

- To ensure that activities are delivered safely, risk assessments are carried out, reviewed, and documented in accordance with the organisations Health & Safety policy.
- To accurately log all client data and activity data into the organisational CRM system - Charity Log.
- To represent Age UK H&W at external events and attend meetings to promote dementia services in the county, feeding back on information delivered.
- Work closely with the Marketing & Communication department and in house teams to promote activities and engage with new members.
- To support the team leader in monitoring and evaluating the service to ensure it is meeting the needs of the members by collating regular feedback through surveys and forms.
- To escalate any concerns or matters relating to risk in relation to a venue, activity, an individual and/or their carers to the Dementia Service Team Leader.
- To build good working relationships with local dementia services including Early Intervention Dementia Service (EIDs), local community groups, statutory and voluntary agencies and social prescribers.
- To ensure that the administrative tasks associated with the role, such as case recording, database entries, statistics, form completion, scanning, customer feedback, etc are completed in a timely, accurate and efficient manner and meet all deadlines.
- To ensure that enquiries/referrals to the service are answered and dealt with efficiently, accurately and in a timely manner.
- To provide cover for the Dementia Meeting Centres in times of staff absence, holiday or increased demand. This may also include completion of assessments, assisting with activities, general kitchen duties and preparing the venue for the day, sourcing and booking entertainment, sourcing and ordering equipment and planning days out which are off site of the meeting centre.
- To provide a safe and social environment (non-clinical) for people living with mild to moderate dementia and their family members.
- To assist the Dementia Services Team Leader
- To work closely with other staff and volunteers to ensure members receive a person-centred approach, including planning and delivering activities that are tailored to everyone’s physical and emotional needs wherever possible. **(This does not include the provision of personal care)**:

- To provide opportunities for members and family members to discuss issues that are concerning them in adjusting to living with dementia and to refer onto the Dementia Wellbeing Service (DWS) for more specialist help where appropriate.
- To maintain the profile of the Meeting Centre and raise awareness of the service within the local community so that it is well-known and accessible and, in conjunction with colleagues in Age UK H&W and other relevant organisations, respond to requests from the local community and referrers for information.
- Undertake general kitchen duties including some food preparation and serving.
- To attend and contribute to staff briefings and relevant training provided by Age UK Herefordshire and Worcestershire.
- To assist with the day to day operations of running the Meeting Centre, including: setting up and taking down the facilities for the provision of the Meeting Centre
- Ensure that all Services are working within their set KPI’s and have the reporting mechanisms in place on the CRM to record
- To assist in the completion and return of new member forms for Age UK H&W and Worcester University
- To cross cover at other Meeting Centres when required.
