The Norfolk Secure Services Forensic Community Mental Health Team provide recovery-focused community care. This is for offenders who have been, or are being, discharged from a secure unit.

This is to focus on service user progression and achieving the best for the service user by reducing the risks of re admission, shorter stays in hospitals and supporting with more community resource to aid recovery.

You will be expected to complete collaborative assessments and review CPA with Service users and their friends, family, and carers.

The MDT is recovery focused and works with a mixed caseload of male and female service users who have complex mental health needs. Working with carers, relatives and other third-party agencies will be a very crucial part of your role.

In addition, this role will focus on supporting reintegration back into the community by in reaching services users placed in inpatient areas and proactively supporting plans to discharge into the community.

By joining our team, you will be well supported to develop further your knowledge and skills to deliver evidence-based interventions for the group of service users. Personal development will be supported through meaningful Appraisal and via a bespoke training package (a comprehensive in house & external training programmes). On a monthly basis you will be offered reflective practice sessions, mandatory clinical supervision, and Line management supervision. You will also have an opportunity to actively take part in Quality improvement projects within the service.
