Associate Mental Health Worker, Discharge

The post holder will work under the supervision of the team/clinical leader. The post holder will promote and encourage healthy lifestyles and will assist people to access the support they need to recover from their mental health and physical health problems, promoting and maintain recovery. They will also support teams to ensure clinical task lists are updated.
The post holder will be responsible for participating and assisting in the delivery of care/services to patients and service users as part of a Multi-disciplinary Community Mental Health Care Team, working under the direct or indirect supervision of a registered member of staff. They will be responsible for carrying out practical tasks, identified as part of the patient’s and service users care plan or care pathways.
The post holder will provide supportive assessment and support to people who have been referred to Oxleas ICMP service. This role will include contacting people to ascertain further information, whether they require time-limited interventions and or signposting as they transition to primary care services.
The post holder will be expected to work within the community team in line with service needs.
The post holder will receive clinical supervision as appropriate and partake in reflective practice/training on a regular basis.
Promoting Health and Preventing Ill Health
- Support people to improve and maintain mental and physical health and wellbeing.
- Involvement in the prevention of and protection against disease and ill health.
- Encourage and empower service users to take responsibility for their physical health.
Provide and Monitor Care
- To work within clinical protocols, guidelines, and the team standard operating procedure.
- Provide care that is person-centred, safe, and compassionate.
- Monitor the health needs of people using positive practice prompts guidance in collaboration with service users.
- Communicate effectively with colleagues and with patients when delivering care or providing advice.
- Recognise and report situations, circumstances or errors which may result in poor health outcomes.
- Ensure information is recorded accurately, in a timely manner on the electronic patient record, and shared when appropriate with health and social care professionals, service users, carers and families.
- Recognise and value service user’s privacy and dignity and ensure the care provided respects equality and diversity.
- Be aware of the need to safeguard service users and follow safeguarding procedures if required.
- Respond to concerns or complaints raised by service users, families or carers and escalate in accordance with Trust polices.
- Maintain confidential information in accordance with Trust Policies.

Undertake a range of duties which involve direct patient care, under supervision following competency assessment.
Assist in the coordination of care delivery for a case load of service users in the community with mental health problems including those with severe and enduring mental illness under the supervision of a registered professional, in close collaboration with the multidisciplinary team, service users and other community agencies. This will include completing assessments of mental health needs of clients.
Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit/community setting.
