SALARY: £22,816-£24,336 - (Starting at £22816 and rising during appointment)

Duties
- To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
- To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the multi-disciplinary team.
- To carry out individual care without direct supervision in the community or clinic setting.
- To have responsibilities for a specific area of work that is facilitated by the Community Matron.

Under the supervision of qualified nursing staff:

- To provide care in accordance with individual service users care plans.
- To promptly report all observed changes in service user conditions, accidents, incidents and other important information to nurse in charge and take action as directed.
- To provide care and assistance in such a manner that the service users individuality, dignity and privacy are respected where this does not compromise safety.
- To assist qualified staff in maintaining the safety and well-being of staff, service users and visitors in accordance with the Health & Safety Policy.
- To participate in the Trust’s staff appraisal system and supervision network.
- To attend and contribute to staff and team meetings as required.
- To assist in the maintenance and management of the supplies and equipment and to report any problems to the appropriate team member.
- To attend and participate in any training courses and events as directed by Department Sisters.

Patient interaction
- Communicate effectively and sensitively and use language appropriate to patient and carer/relatives condition and understanding.
- Effectively use all forms/methods of communication and be aware of and manage barriers to communication.
- Effectively recognise and manage challenging behaviours in patients, carers and or relatives

Health and Wellbeing enablement
- Provide information to team and patient, carer and or relatives
- Respect the dignity of individuals, obtaining consent and involving them, their carers and or families and relatives in shared discussion making.

Clinical skills
- To effectively and efficiently undertake all aspects of clinical duties and procedures required and identified in an individuals’ treatment plan, recognising own competencies and the scope of own role. Such duties/procedures* will include:

- Urinary management
- Bowel management
- Continence management
- Chronic disease management
- Palliative and terminal care
- Aural care
- Baseline observations
- Life support in line with Resus Council guidelines
- Implementation of rehabilitation activities/exercise programmes
- Therapeutic equipment/aid supply, instruction and supervision/evaluation of use.
- Supporting people during clinical procedures.

**Requirements**:
Physical
- Be able to react to emergency situations in an unpredictable working environment
- Standing/ walking and sitting - often for long periods
- Irregular breaks
- Shift patterns

Emotional
- Dealing with complaints from service users
- Stressful environment when Department busy
- Reacting to emergency situations when presented
- Adult, Child/Adult Protection issues

Mental
- Need to be able to multi-task
- Concentrating for long periods of time
- Need to be able to multi-task in the decision making process
- Interruptions to concentration

Working Environment
- Frequent exposure to all bodily fluids and unpleasant odours
- Contaminated equipment

Skills and Attributes
- Good verbal communication skills
- Good literacy skills
- Well-developed inter-personal skills
- Able to keep accurate records
- Flexible to meet demands of the service
- Able to work shifts over a 24 hour period and work weekends and bank holidays.
- Able to travel across the county to attend essential training and other required training for your role
- Willingness to work on other wards if required
- Group work experience
- Leadership experience
- Current Driving Licence
