Develop a positive relationship with service users and gain a comprehensive understanding of their interests, likes, dislikes, communication methods etc.
- Responsible for contributing to the continuous updating of detailed, person-centred Care Plans and positive behaviour support plans.
- General housekeeping e.g. cooking, shopping, domestic duties etc, and helping Service Users to maintain all aspects of their domestic lives.
- Develop service users’ independence and life skills within their home and out in the community.
- Providing help with all aspects of personal care ensuring dignity and respect
- Working with other health and social care professionals to provide and maintain person-centered care and development plans.
- Monitoring progress and keeping records.
- Liaise with families, friends, advocates, external agencies, and the management team to provide a consistent approach that meets the individual needs of each service user.
- To empathise and actively listen in a sensitive manner accessing appropriate additional support where necessary.
- To support Service Users to access appropriate health care provision enabling them to make an informed choice regarding any prescribed treatment, seeking advice where necessary.
- Liaise with staff members and management about any issues that need addressing with service users
- Complete relevant paperwork at the end of every shift. Personal hand over sheets, personal care check lists, behaviour records, etc.
- To create opportunities for Service Users to develop establish and maintain personal relationships and social networks, which encourage greater participation and integration in their chosen communities.
- Helping to organise leisure activities.
- Food preparation and administering medication.
- To participate in regular training relevant to individuals needs and policies/procedures.
- The ability to work as a team.
**Job Type**: Part-time
**Salary**: £10.42 per hour
