**About us and the role**

An exciting opportunity has arisen for a Commissioner to join the Coventry and Warwickshire Integrated Commissioning Team for Disabilities and Autism, on a fixed term contract for 1 year. This post is part of an integrated team working across Coventry and Warwickshire and Health and Social Care, hosted by Warwickshire County Council on behalf of Coventry and Warwickshire Integrated Care Board, Coventry City Council and Warwickshire County Council.

The Integrated Commissioning Team is responsible for delivering the local commitment to improving the lives of children and adults with a disability and autistic children and adults by helping people to be more independent, have healthier lives, build strong networks in their communities and have more choice and control over their care and support. This work remains a national and local priority.

We are looking for a dedicated and enthusiastic individual, with a passion for commissioning high quality care and support solutions, to undertake this role. The post-holder will be responsible for the delivery of specific integrated commissioning activity, including operationalising, monitoring and assuring delivery of service specifications across. Examples of provision that the postholder’s activity will focus on includes, but is not limited to: Inpatient Admission Avoidance and Discharge provision (for people with LD and autistic people) including the development of new models of care and support, the Coventry and Warwickshire Keyworker Service, developing initiatives to further develop the local provider market/associated workforce and health based respite provision.

This commissioning role will require you to work effectively with a range of stakeholders, internally and externally, have excellent project management skills, and have experience of commissioning services working in partnership with people who access them. This is an exciting opportunity particularly if you are interested in developing your knowledge and experience of integrated working across health and social care.
