The post-holders will work together with a range of colleagues to improve the experience of people with complex long term neurological disabilities, as they journey through the health and community care service system. The neuro-navigator’s key role is to provide direct support to the acute and specialist rehabilitation services, with expert clinical advice, assessment and education, and directing patient referrals to the most appropriate specialist service. They will provide an overview of the neuro-rehabilitation pathway, supporting patients with neurological rehabilitation needs, as they move from acute services through to in-patient services and onto community services, including within the NHS continuing healthcare framework.

This post would be ideal for someone who has experience in the neurorehabilitation or neurology field and who is passionate about high quality clinical care and supporting patients and their families during this critical time.

The Neuro-Navigators will be based within the North Wales Brain Injury Service, an inter-disciplinary community-based neurorehabilitation service, based at Colwyn Bay Hospital, and will work in conjunction with key clinicians within the neurological rehabilitation pathway, the Individual Patient Funding department (IPFR), and continuing healthcare teams (CHC).

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
