An exciting opportunity has arisen for a Health and Wellbeing Coach to join our Health and Wellbeing Team in Easington Central PCN.

In your role as a Health & Wellbeing Coach, you will work as part of the Easington Central PCN Integrated Personalised Care Team, alongside 3 other SPLWs and 2 Care Co-ordinators. The posts are hosted by one practice but will support patients across the three practices within the PCN.

We aim to provide person-centred care, promote health and independence, including use of care navigation to other professionals where appropriate.
