An exciting opportunity has arisen for a SPLW to join our Health and Wellbeing Team in Easington Central PCN.

In your role as a SPLW, you will work as part of the Easington Central PCN Integrated Personalised Care Team, alongside our SPLWs, Care Co-ordinators and heath well-being coach. The post is hosted by one practice but will support patients across the three practices within the PCN.

We aim to provide person-centred care, promote health and independence, including use of care navigation to other professionals where appropriate.
