**Job Title: Dawlish Hospital SupportWorker**

**Normal Working Pattern: 16 - 20 hours per week, Mon to Friday**

**Location: Dawlish Hospital**

**Responsible to: General Manager**

**Main Purpose of Role**
- To support patients at Dawlish Hospital to have a safe and timely discharge

**Key Tasks and Responsibilities**
- Attending daily handover meetings to discuss clients’ needs before visiting them, and to feed back on their needs after meeting them - Monday, Wednesday, Thursday and Friday at 10am. At the meeting you will get the ward ‘Handover’ sheet and with ward staff go over patients on the ward; medical needs/details of their stay/other professionals involved/any VIH involvement and discharge requirements. The handover sheet is your guide for the day.
- Attending weekly multi-disciplinary team meetings, MDT, is on Tuesday 2.30 -3.30 it is important we attend - to add our contribution if we are working with the client/make requests on behalf of the client/add any further information if VIH is to be part of the discharge plan/make appropriate signposts.
- Visiting clients and their families on the ward to offer support
- Signposting to a range of services (meal deliveries, befriending, transport etc) from Volunteering in Health and other local organisations
- Telephoning clients 48 hours after they have been discharged to see if they need any further support
- Completing monthly statistics including one case study to show how our work is helping clients be discharged more safely and quickly than they would otherwise be
- Working with the NHS Discharge Co-ordinator and other Hospital Staff effectively
- Working with the rest of the Volunteering in Health team to offer a seamless service when a client is discharged from hospital
- Attending Volunteering in Health team meetings and supporting the rest of the team as required
- Organize a structure and a way of working to ensure - all patients are seen, charity log updated, stats provided at the end of each month, sign posting knowledge of services inside an outside of area is kept up to date, appropriate referrals are made to VIH for support including follow up discharge calls.
- Keep leaflets and signposting info up-to-date and support ward staff.
- Keep notice boards up-to-date with relevant info.
- Keep food bags and ‘Patient Support Provisions Hub’ fully stocked according to needs - expand the ‘Patient Support Provision Hub’ to support other related professionals in the area. Keep data and experiences of how this hub has supported others.
- Complete a ‘Home from Hospital Referral Form’ if connecting patients to other charities within the Wellbeing Partnership.
- All confidential documents need to be locked in the draw in the VIH room or put into the NHS confidential waste.

**Targets**
- Visit every patient on the ward during their stay and offer them support as appropriate
- Attend handover and MDT meetings every week, normally Tuesday afternoon
- Completing monthly statistics each month

**Job Type**: Part-time

**Salary**: From £12.38 per hour

Expected hours: 16 - 20 per week
