This post is to join a team supporting working-age adults who have moderate to severe affective and psychotic disorders. The team offers care-coordination, a daily duty service, psychiatric assessments, medication reviews, ongoing risk management, care planning, support around benefits and housing, family work & a wide variety of therapeutic interventions.

We actively develop our staff in their ability to deliver specific interventions. We are continually developing the service, considering what interventions may be helpful for our service users, & evaluating the support we already provide. Recovery principles are embedded in the work of the locality teams & we aim to support people to reach their potential by facilitating hope, empowering autonomy, and developing meaning & purpose in life.

There is a requirement to be able to travel around the Cambridge area.

You will be involved in the delivery of care plan interventions under supervision from qualified practitioners in the team.

You will support around social inclusion for service users; much of your work will be out and about in the community accessing mainstream community services and networks.

You will offer support around benefits and housing, carers’ assessments, physical health care and a wide variety of therapeutic interventions for which you can access training and supervision.

To support service users with their recovery journeys through collaborative goal setting with a range of priority health and social needs under the supervision of a senior member of the team.

Work with service users to facilitate access and engagement in a range of socially inclusive (including vocational) community based activities.

Develop an alliance based upon collaboration and mutual respect and within appropriate and transparent boundaries.

Help users gain access to resources to include benefits and welfare rights, using internal and external expertise as required.

Help to identify early signs of relapse by monitoring the user’s progress, level of functioning and mental state and alert the appropriate staff involved in the individuals care.

Provide simple psychological interventions as required under the supervision of an appropriately qualified clinician.

Participate in regular planning and review meetings with the clinician supervising pieces of work and maintain quality and management of clinical caseload within supervision process (pieces of work may be supervised by more than one team member).
