The Quality Assurance and Safety Team aims to support services and directorates to continuously monitor patient safety and improve the service being provided to staff and patients. We all want to ensure that both staff and patients get the best experience when accessing services provided by Hywel Dda UHB and when working in the Health Board.

This includes quality assurance through mechanisms such as WalkRounds, spot checks and inspections and all aspects of incident reporting, incident investigation, incident notification and supports the Health Board as a Learning Organisation.

The post holder will need to use DatixCymru, monitor progress with incident investigations, escalate concerns regarding patient safety, liaise closely with senior managers and teams in directorates and services, provide training and report on themes, trends and outcome from the tools in the quality management system.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

The post holder will be a member of the Quality Assurance and Safety Team. They will be responsible for supporting the Patient Safety and Assurance Manager and working closely with the Patient Safety Officers in the management and governance of patient safety matters including investigations, provision of data and support of directorates in their programmes of work to improve the safety culture of the organisation.

They will also assist in co-ordinating and overseeing the management of the investigation process of Serious Incidents and high-level Incidents, and that the reporting, recommendations and any practice changes that are required are carried out to bring about change. They will work as part of the team to support learning, quality improvement and positively impact the patient safety culture within our organisation. The post holder will support directorates and service based colleagues on the identification and management of patient safety.

There is an element of training that is required of the post holder; the team is responsible for incident investigation training, training for managers on the incident reporting system and training for all Health Board staff about patient safety.
