Lung Nodule Tracker/pleural Co-ordinator

We are looking for enthusiastic and motivated individual to join the Lung Cancer team as a Lung Nodule Tracker/pleural co-ordinator to support the implementation of faster Cancer Diagnosis Standards and monitor patients who are on the Lung Cancer/pleural pathway. This is a new permanent role with an exciting opportunity to test new ways of working to make a difference with faster diagnosis and to deliver quality patient care.

The post holder will be an integral part of the Lung Cancer /Pleural Team, to assist, support and co
- ordinate the lung nodule pathway for patients. A large part of the role is providing administrative, tracking and coordination support to patients, families, Consultants, Cancer Nurse Specialists and other staff involved in implementation and monitoring of The Faster Diagnostic Standard (FDS).

Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which will commence in 2021

We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

To support the clinical teams in the Care Co-ordination patients on Lung Nodule/pleural Pathway in accordance with National and local guidelines.

To establish and develop communications with other departments, including Imaging to facilitate CT Scans and to ensure CT scans are performed and reported timely

To liaise with tertiary centres, ensure PET scans are transferred and available for meetings

To collect, collate and input all data required for Patients on Lung Nodule/pleural Surveillance.

To facilitate smooth and efficient patient pathways utilising IT systems and Data bases

Co-ordinate virtual clinics, informing patients they will be receiving telephone consultation

Facilitate and provide administrative support for Lung radiology meetings
- take minutes. Ensure all relevant information is available for the meeting

Work collaboratively with the Lung Cancer Specialist Nurse to facilitate from lung nodule pathway to Lung cancer pathway for enlarging Pulmonary nodules.

Facilitate patients from lung nodule pathway to the Trust Cancer Pathway by liaising with Cancer services to upgrade patients on to cancer pathway.

Provide administrative support and facilitate out patients appointments.

Provide administrative support for Consultants.

Communicate non-complex outcomes to patients
