**About us**

Newfield Medical Group are pleased to be in a position to advertise for a Practice Community Coordinator. We are seeking a passionate and committed individual to take a lead in expanding our community work. You will be responsible for establishing and supporting the development of a community hub working partnership with third sector organisations. You’ll be an enthusiastic, energetic and forward thinking person with the ability to lead the Community team, including volunteers, to develop a GP practice linked community hub.

**Job role / purpose**

The post holder will establish and develop community services within a GP practice based hub and develop the newly formed Newfield Community Group (NCG) SCIO in line with its charitable purposes. The post holder will be responsible for managing and operating a community hub linking local residents with a variety of services. You will review the needs of the area regularly and have the ability to set up new projects meeting identified objectives as determined by the board of trustees of NCG whilst being accountable to the Board of Directors of Newfield Medical Group Ltd who are establishing this post to support the charity (NCG). You will be an ambassador for both the charity the practice.

**Key duties and responsibilities**:
**Establish, develop and facilitate a GP practice based community hub**

Establish, develop and facilitate a range of community based services helping to address the needs of the local population.

Take a lead in developing the range of services available locally, dove-tailing with existing services and introducing new services to meet identified needs.

Working in partnership with relevant providers and agencies to provide services appropriate to the needs of the community.

Identify need, fostering strategic partnerships to respond to needs presenting new project proposals to the Board of Directors.

Managing relationships with key supporters, media, local and national government and other key hub stakeholders, especially their community.

Have knowledge of the community served and using appropriate strategies reduce barriers and promote participate from all within the community.

Undertake day to day management of hub staff providing regular support and supervision.

Undertake recruitment, day to day management and continuing development of hub volunteers providing regular support and supervision.

Provide project management including planning, delivery, monitoring and evaluation in line with guidance from Trustees.

**Health and safety**

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures. This will include:

- Using personal security systems within the workplace according to practice guidelines
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
- Making effective use of training to update knowledge and skills
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards, keep up to date with food standard requirements and ensure standards are adhered to in line with local council as the project develops.
- Actively reporting of health and safety hazards and infection hazards immediately when recognised
- Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role
- Reporting potential risks identified
- Demonstrate due regard for safeguarding and promoting the welfare of children.

The post holder will be responsible for ensuring all Health and Safety regulations and risk assessments for the hub are carried out according to appropriate guidelines.

**Representing the practice**

Representing the Practice on relevant strategy groups and other local and national forums.

**Patient confidentiality**

Adhere to the strict practice patient confidentiality policy

**Social Media**

Adherence to the practice’s strict social media policy. No practice or patent matters to be published on social media or responded to on social media.
