**Job Description Job Information Job Title**: Home Parenteral Nutrition Nurse Directorate / Service: Gastroenterology and Hepatology Care Group - R Homecare HPN Service AfC Band: 6 Accountable to: Nurse Consultant in Nutrition Reports to: Team Leader Base Location: R Homecare Office, Broadgreen Hospital AFC Job Code: ESR Position Number: Job Summary The post holder will work as part of a team of nurses dedicated to Home Parenteral Nutrition (HPN) patients. They will require clinical skills and expertise in the assessment, preparation and training of patients and their families for Home Parental Nutrition, Intra-vascular devices and Intestinal Failure management at home. They will undertake the follow up of patients in the home and appropriate outpatients clinics. They will give clinical advice to patients about their Home Parental Nutrition related problems.

The post holder will be able to work unsupervised with the support of a Team Leader. Key responsibilities CLINICAL RESPONSIBILITIES To have own case load with the support of the team leader. Assess patients as to their suitability, the suitability of their environment and the social support available, for various treatment options. Promote a healthy and appropriate lifestyle.

Be a link between the hospital and the community. Refer patients to appropriate agencies. Liaise with the multi disciplinary team in the hospital and appropriate Primary Care Trusts. Supervise patients and their relatives in their home environment.

Undertake Home Parental Nutrition administration and associated therapies in the patients home as required. Support patients and relatives through their care. Attend relative clinics giving specialist input. Attend pre assessment clinics prior to the patients discharge on Home Parental Nutrition.

Undertake clinical procedures and tests in the patients home. Handling bodily fluids when necessary. Working in different conditions on occasions, adhering to the lone worker policy. Administer Intra-venous Therapy following protocols.

Problem solving Be aware of co morbid factors that may influence the patients care. Counsel patients and their relatives with regard to being told bad news. Use bereavement counselling skills. Participate in clinical audits.

EDUCATIONAL RESPONSIBILITIES Inform Home Parental Nutrition patients and their relatives as to treatment options available. Teach staff, both hospital and community based, about the Home Parental Nutrition role. Teach staff in other hospitals to undertake various procedures. Be a link nurse to the multi disciplinary community team if necessary.

Train patients to undertake connection and disconnection of Parental Nutrition and other related procedures. Facilitate the support of patients and relatives. Assist in any new expansion within the directorate. Keep updated in all areas of I.T.

Develop both personally and professionally. PROFESSIONAL RESPONSIBILITIES Liaise with the multi disciplinary team to maintain good communication. Liaise with the medical team to monitor optimum patient care. Establish and maintain links with other specialist nurses.

Be the patients advocate. Report to the directorate clinical issues surrounding clinical governance and/or risk management. Maintain own professional development. Manage own time and resources.

Have the necessary skills with regard to communication via telephone and I.T. Help with the making of protocols and policies. Participate in clinical supervision. Maintain accurate patient records.

To audit role on a regular basis so as to maintain a high professional standard of care.
- The post holder shall as necessary provide cover for and undertake duties of absent colleagues.
- The post holder shall follow all the policies and procedures of the organisation.
