**Job Purpose**:
- To improve the lived experience for people living with dementia and their carers in line with the Greater Manchester Dementia United Standards.
- To provide practical and emotional support to assist carers and people living with dementia to develop coping strategies in order to relieve their caring responsibilities.
- To provide a targeted, responsive, information and advice service to BAME people with a diagnosis of dementia their carers, families and friends.
- To participate in local events and workshops.
- To raise awareness of Dementia and it’s impact on families supporting them to understand and navigate the Care Pathway through information, advice and signposting.
**PRIMARY DUTIES & AREAS OF RESPONSIBILITY**
- To promote the ACCG Dementia Advice Service and develop networks with partner organisations who also work in the associated field of dementia, i.e. mental health, disabilities, benefits and income advice, including Primary and Secondary care, Social Care and Health, and Welfare Benefit agencies, with the aim of promoting control and empowerment for individuals to make informed and better choices.
- To help people to navigate and access appropriate services, ensuring they are able to make contact with the right organisation/person that is able to help them.
- To facilitate the weekly Dementia Advice, drop-in service.
- To establish and maintain a Dementia support Network for people living with dementia, their families and carers with the aim to reduce social isolation by encouraging interventions such as attending group activities and workshops.
- To support service users to remain in the community through practical advice assisting with the removal of barriers to access support for their needs
- To gather all appropriate information from the service user and their carer to enable the appropriate guidance and advice to be offered.
- To provide on-going practical support as required throughout the person’s life as they live with dementia.
- To ensure that service users and their families have access to translation and interpretation services as appropriate to their cultural and religious needs
- Working in partnership where necessary with other organisations, to ensure that the service provided responds fully and effectively, promoting control and choice of service users.
- To be responsible for the training and supervision of Dementia support volunteers
- To keep updated information on available resources from health and social care agencies and other organisations. This will enable the Dementia Team to provide high-quality information and advice.
- To record information as required within the ACCG database in order to monitor and evaluate the effectiveness of the service from contacts and user experiences.
- To be the first point of contact for Dementia support information and advice.
- To work with the person with dementia and their carers, families and supporters to formulate a support and information plan.
- To represent ACCG at community events.
**Job Types**: Full-time, Temporary contract
Contract length: 24 months
**Salary**: £21,748.00 per year
