Supported Living Care Coordinator with Strong

We are looking for hands-on Care coordinator with strong communication and documentation skills. The day-to-day activities will include the following:
- Deputising for the Registered Manager
- Handson experience in developing Care/support plan, health and activities plan, carry out risk assessment
- Excellent written and communication skills
- To make sure all provision is in line with CQC regulation
- File auditing and KPI monitoring
- Start-up of new packages of care with ongoing monitoring and review
- Client satisfaction reviews
- Care worker spot checks, supervisions and appraisals
- Recruit, select and effectively supervise staff teams
- Carrying out visits to client’s homes, hospitals/care and nursing homes to carry out initial client assessments
- Monitoring and maintaining weekly rostering and medication alert database.
- Actively participate in the growth and development of the business
- Hands-on care when required
- On-call responsibilities
- Preferably have QCF level 5/Level 4 NVQ NVQ level 3 in Health and Social Care
- Preferably experience working with people with learning disabilities and sensory needs
- 2 years domiciliary care experience
- 2 years supervisor/ management experience
**Job Types**: Part-time, Temporary
Contract length: 6 months
Pay: Up to £15.00 per hour
