Senior Community Practitioner Intensive Home

We are looking for a dynamic health professional to join our established team to support patients at risk of health deterioration within their own home. This will require working across a Pennine Lancashire footprint to provide clinical support to the single point of access and respond to patients who are triaged and require a 2-hour urgent response.
You will initiate interventions by providing a holistic advanced assessment which may include non-medical prescribing and enhanced clinical skills to support patient safety and reduce risk.
Established in 2015, the service continues to evolve to meet the needs of a diverse population with the aim of providing complex clinical interventions within community settings.
The Intensive Home Support Service were pivotal in the set up and implementation of the Covid Virtual Ward in response to the Covid pandemic which has proved hugely successful and the development has subsequently been shortlisted for a HSJ award
The post holder will provide advanced clinical assessment, needs identification, planning, case management and co-ordination of care for people with long term conditions who have highly complex needs and co-morbidities.
The post-holder will promote partnership working to improve outcomes for patients with co-morbidity & long term conditions in order to reduce unplanned hospital admissions, reduce length of stay, facilitate seamless transition of care and provide post discharge support to prevent avoidable readmission to hospital.

As patients are living longer with more complex needs, the NHS has adapted to meet this growing need; ensuring patients have more choice, equitable access to services, and their needs met in their usual place of residence. This allows for hospital admissions to be reduced, creating capacity within our hospitals and emergency departments, to ensure that patients’ needs are met at the right time, by the right person and in the right place.
ELHT & Pennine Lancashire are expanding their out of hospital services and growing their award-winning Intensive Home Support Service, including hospital at home pathways. IHSS has medical cover to support acute interventions: IV therapies, ultra sound scanning, ECG and point of care testing.
IHSS is an integrated multidisciplinary team operating 24/7 via varying shift patterns. This service is supported by a dedicated single point of access (SPoA) Intermediate Care Allocation Team (ICAT) comprising of social care coordinators and other health and social care professionals
MAIN DUTIES
- Co-ordinate comprehensive programmes of care that promote health gains, reduce risks to health and well-being, short term and long term and maximise independence of client and patient with co-morbidity long term conditions. This will include:
- Health and social assessment including medical history and clinical examination.
- Diagnosis and exercising independent prescribing in accordance with appropriate legislation and ELHT protocols.
- Functional/cognitive assessment.
- Ensuring that client values, beliefs and views, are reflected in their management plan.
- Co-ordinate care and treatment to avoid fragmentation, duplication and delay in the least intensive setting appropriate to the client’s needs by:
- Requesting and interpreting diagnostic investigations in order to implement and review a programme of care to meet the needs of the client.
- Referring to specialist services.
- Ensuring effective communication and sharing of appropriate information amongst professionals to avoid conflicting treatments.
- Identifying deficiencies in service provision and addressing these appropriately.
- Use developing skills and knowledge to instigate therapeutic treatments consistent with best available evidence with appropriate input from multidisciplinary teams and other agencies.
- Be proactive in the rigorous monitoring and evaluation of the effectiveness of agreed care management plans, providing timely interventions and modifications as necessary in partnerships with clients/patients.
- Act as a first point of contact for identified patients with an acute episode, triaging calls and offering telephone advice, home visit, or referring on to other appropriate professional/service.
