**Learning Disability Care and Support Worker Job Description**

**Our Services**

We offer day time services that encourage individuals with learning disabilities and/or autism, including profound and multiple learning disabilities, to take part in meaningful opportunities and activities in Taunton. We respect the individuality of each person and offer the freedom to express their personality and make informed choices about their care. We pride ourselves on our person centred approach and strive for excellence in all areas of service delivery.

**Job Summary**

To be creative, innovative and engaging while delivering care and support. Promoting a supportive, nurturing and caring environment, which provides customers with innovative and meaningful ways to enjoy their days with us. This will require a high standard of creative and person centred planning, meeting individual needs, wishes and aspirations while ensuring everyone is treated with dignity and respect. Maintaining independence and ensuring that rights to privacy and choices are met.

To undertake administration in relation in all areas to a professional standard. To be creative, innovative and engaging in all aspects of delivering their bespoke care and support plan.

You will be expected to carry out training in line with the requirements of the role and work together in a close-knit team to deliver personalised support including coming up with ideas/suggestions, session planning, activity delivery and all aspects of personal care (as required).

**Main Tasks and Responsibilities**

**Customers**

This is a role where **your contribution counts** - helping individuals to achieve their personal aspirations, being a role model to encourage positive choices and ensuring that the individuals achieve maximum benefit from their time with us.
- Actively engage with the customer, their families, and other professionals to ensure our services are safe, meaningful and enjoyable.
- Contribute to planning and implementation - to ensure activities are properly researched, appropriate, meaningful and in line with customer preferences. Activities may be at our centre in Taunton or enjoying the community.
- Encourage individuals to make choices for themselves, develop new skills and interests.
- Support with personal learning, trying new things
- Support to understand risks, the impact of taking them and documenting decisions.
- Maintain customer’s records and plans.
- Support in personal care (as required), whilst promoting independence and ensuring dignity is upheld.
- To actively engage in customers chosen activities and never be a by-stander.
- Contribute to the regular reviewing and updating of the personalised support/care plans and risk assessments.
- Seek guidance from senior members of staff if specialist advice is required or if you have any cause for concern.

**Staff**
- Attend all mandatory training sessions and follow up refresher courses as required to fulfil your role.
- Be aware of and abide by all relevant legislation and guidance particularly the Mental Capacity, Care and Equality Acts.
- To be fully aware of all areas of the Underpinning Knowledge handbook and contribute to its further development.
- Participate in regular 1:1’s/supervisions/appraisals and team meetings.
- Maintain a positive and professional image of yourself and the organisation including a high standard of personal hygiene.
- Comply with all company policies, procedures, directives and instructions.
- Promote, and act at all times - in accordance with the Equality, Diversity and Inclusion Policy.
- Maintain up to date daily records and prepare any required reports. e.g. incidents, accidents etc.
- Ensure care plans and daily logs are completed in a timely manner.
- Manage time efficiently and effectively.
- Understand and conform to national and local policies, responding appropriately to emergency procedures and the actions required in the event of an incident or accident.
- To be vigilant to potential safeguarding concerns and respond in accordance with the Safeguarding Vulnerable People Policy and Procedure.

You must have full awareness of the confidential nature of this post. Disclosures of confidential information or disclosures of any data of a personal nature can result in prosecution for an offence under the Data Protection Act 2018 or an action for civil damages under the same Act in addition to any disciplinary action.
