**Job Summary**

The social prescriber link worker (SPLW) works alongside a team within general practice and within the Primary Care Network (PCN) and empowers people to take control of their health and well-being.

Social prescribing can help to strengthen community resilience and personal resilience whilst reducing health inequalities and can be particularly beneficial to patients with long-term conditions, those with mental-health issues and those who are lonely or isolated or who have complex social needs which affect their wellbeing. A referral to a non-medical ‘link worker’ is designed to support patients in being able to take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

The post holder will be an integral part of the general practice team. Working closely with GPs, GP practice teams and other health and care professionals across the PCN, As well as part of wider community groups, the PCN’s multidisciplinary teams, the Health & Wellbeing and Care Coordinators to promote and embed a personalised care approach.

You must be able to work independently as well as part of a team, be a forward thinking, caring, dedicated, reliable and person-focused individual who enjoys working with a wide range of people. You will have a strong working knowledge of Office 365 programs and general IT, good written and verbal communication skills and proven strong organisational and time management skills. We are looking for highly motivated and proactive applicants with a flexible attitude, keen to work and learn as part of a team, and who will be committed to providing patients with high quality support.

Although this is predominantly a remote working role, you will be expected to attend in person, monthly team meetings and on a rota basis our face to face sessions. With this in mind the post holder will demonstrate excellent organisational and communication skills, be flexible in their approach, able to exercise initiative and demonstrate consistently high standards of professionalism. The post holder must at all times be aware of the need for confidentiality and integrity. A basic knowledge of Health and Social Care terminology, structures and pathways would be a strong advantage.

**Mission Statement**

Primary Care Networks (PCNs) are a new way of working that enables health and other services to work together to provide better access for patients.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

The Meridian PCN is a collaboration of six GP surgeries across Cambridgeshire and North Hertfordshire; supporting a patient population of +48,000.
