Responsible for coding of relevant information to the patients electronic healthcare records. Read all incoming correspondence to the practice, accurately coding the relevant diagnosis/procedure/information into the patients records. Ensure that follow-up actions/ tests are brought to the attention of the relevant staff. Scan patient-related documents onto their medical records using codes as agreed by the clinical team.

Ensure incoming letters are scanned onto the patient record. Ensure all QOF related data is captured. Ensure that all clinical codes are current. Ensure that patient medical problems are grouped and combined appropriate so that the medical records are always tidy.

Follow practice protocols to ensure consistent and high-quality care is provided to our patients. Correct errors in filing and ensure corresponding records are amended accurately.
