Community Connections provides support for young adults and older people who may be experiencing isolation and loneliness or a limited social life. It is a free service which aims to encourage people to be more socially active and to build happier and stronger relationships.

**Vacancy**: Wellbeing Worker within Community Connections

**Hours of Work**:21 hours - weekdays, Monday, Tuesday and Thursday

**Salary**:£12.10 per hour (£24,330.40 pro rata, £13,213.20 actual per annum)

**Contracted post**: Fixed term contract until 30th June 2025

**Base**:Brian Jackson House, Huddersfield however this role will involve significant work within the community and travel within the Kirklees area.

- **Purpose of the role - why the role exists and overall responsibility**_
- To support the Service Manager and team with the development and implementation of YCC’s Community Connections Services which aims to reduce loneliness and social isolation for vulnerable adults across Kirklees
- To improve the wellbeing of adults accessing these services and to ensure target communities needs are met
- **Job Description - Main Activitie**_**s**
- To complete service assessments for adults that have been referred to Community Connections as part of the Befriending Service
- To provide effective and supportive communication with service users and volunteers
- To match appropriate volunteers with appropriate service users
- To provide ongoing support to volunteers
- To work closely with service staff, partners and relevant agencies and any other agreed parties to help to improve the wellbeing of service users and volunteers
- Contribute to the monitoring and evaluation of the service to capture outcomes
- Record activity and outcomes for volunteers and clients in line with contractual /grant requirements
- Facilitate meetings in line with service requirements
- To attend and work with colleagues to support, organise and promote networking or other associated events (internal and external)
- Provide regular and ad-hoc reports to the service manager as requested
- Adhere to all relevant YCC polices and procedures
- Maintain and pursue personal development of skills and knowledge necessary for the effective performance of the role i.e. knowledge around Safeguarding
- Demonstrate all YCC values during the course of your role
- **Knowledge, training, and experience required**:_
- Excellent verbal and written communication skills
- A relevant professional qualification or experience in: youth work, teaching, counselling, or another appropriate field
- Knowledge and understanding of issues affecting vulnerable and older adults at risk in relation to both health and social care
- Prior experience in working with clients with challenging behaviour, or clients that are seeking access to support services or requiring information and guidance would be desirable
- Experience and knowledge around Safeguarding practices relating to vulnerable adults would be desirable
- Experience of developing and maintaining relationships with a wide range of partners would be desirable
- Good geographical knowledge of Kirklees and access to a vehicle
- GCSE/Level 2 English and Maths or evidence of a good standard of numeracy and literacy
- Experience of using computer packages including word processing, databases and spreadsheets

- Effective communicator, who can engage and respond to the needs of a wide range of people, e.g, service users, colleagues and professional associates

- Ability to prioritise own workload, and work flexibly to achieve individual and team goals
- Ability to reflect on practice and identify or implement changes to support and improve individual or team efficiency and effectiveness
- The ability to employ good listening skills, and is tactful and considering of the needs of other when communicating.
- Takes pride in working to high standards and working within a team supportively to achieve collective targets

**Job Types**: Part-time, Fixed term contract
