NHS Lincolnshire CCG has developed an Elective Activity Coordination Hub (EACH) to support patients in accessing the most appropriate appointment for their care. The role of Patient Referral Service Facilitator involves working in a team using an electronic system (eReferral Service) to book patient’s appointments for further tests/treatment once they have seen their GP. You will be expected to liaise with patients by telephone to ensure suitability of the appointment and to give them any necessary information they require to make an informed choice (current waiting times for example).

You will utilise a software package to manage the patients within the system; including undertaking admin checks to ensure all relevant information is present, scheduling the patient for clinical triage and then liaising with the patient by telephone to book them the most appropriate appointment. The service works to strict deadlines and requires you to handle confidential, complex and sensitive patient’s clinical information on a daily basis. You will also be required to communicate with other partners (GPs, Providers) who provide the direct patient care.

The Referral Facilitation Service forms part of the Elective Care Hub which is responsible for navigating patients and providing choice in their journey between presenting to their GP and them requiring specialist consultation and/or treatment.

Main duty is to receive an electronic referral for a patient, undertake an admin check to ensure all relevant information is present, schedule the patient for clinical triage and then liaise with the patient by telephone to book them the most appropriate appointment.

Also supporting the system validating some of the longest waiting patients and reallocating to other providers
