**We are currently recruiting **

A motivated and decisive **Social Care Support Officer **to work within the **Intermediate Care & Allocations Team **in **Adult Social Care (ICAT) **at **Lancashire County Council.**

ICAT are a team of social care professionals working with the hospital and in the community. They are committed and focused to support with arranging safe efficient hospital discharges, and complete social care reviews. They also support with the avoidance of hospital admission for our service users wherever possible by acting swiftly to assess necessary care. As our new Social Care Support Officer, you will be working closely with a multitude of services to ensure the best possible outcomes for individuals in hospital settings and at home, by assessing service users, their carers care, and equipment needs. We want you to bring bundles of enthusiasm and empathy to the team, adding value to the work we do by taking a person centred and strengths-based approach.

**The post on offer is a Permanent contract based on a 7- day working pattern so there will be a need for you to work some weekends.**

**What we are looking for **

**Passion **- you'll be passionate about making a difference to people's lives, determining the right level of care and providing superior holistic assessments - we want you to be autonomous and an advocate for our service users, forming a worthwhile connection with your calm and collected demeanour - you will be supporting with early discharges from hospital as well as visiting people's homes to assess various situations, so we need you to be courteous and respectful in every setting - your attention to detail will be second to none, identifying any and all risks to prevent hospital admission or any future health related issues

**Challenge **- you will be faced with a whole host of different needs and issues, not knowing what you will be faced with when working in the community - it is important that you can make those tough decisions, remaining firm but fair when it comes to determining what interventions are required - we want you to be insightful and have an awareness of the resources and services around you to mitigate any risks - you will be required to cover Blackpool Victoria and Clifton hospital as well as Thornton and Fylde and Wyre, so we need someone with a get up and go attitude - the workload is never something you can plan for so you will need to be prepared for anything, demonstrating resilience and thinking on your feet

**Collaborative **- although you will be working independently and need to be comfortable doing so, you will also be working alongside different Health and Social Care professionals - this is a well-respected and established team which has a primary and critical focus around multi-disciplinary working - your input and support will always be valued as you work closely with general practitioners, wards, relatives, community health and so much more - by joining us you will benefit from a comprehensive two-week induction programme in our bespoke Adult Social Care Academy, as well as regular supervisions with managers and opportunities to enhance your skills and knowledge
