**Purpose**

To fully utilise an Assessment, Planning, Implementation and Review approach within Service Delivery based activities in order to equip Service Users with the knowledge, skills and confidence they need to enable them to pursue their journey of choice towards living independently ensuring support networks are available to Service Users by working in collaboration with partnership agencies.

**Key Tasks**

2. Signpost Service Users to enable them to meet their desired outcomes and aspirations and to promote partnership working.

3. To provide a quality service and response to Service Users at all times by continually promoting strong engagement with Service Users, encouraging them to link in with Internal and External Service to achieve their full potential.

4. To work alongside Housing Management workers effectively and efficiently in order to manage all areas of Housing Management throughout the LINKS provision with particular emphasis on Maximising Income and Minimising Loss for the Association.

**Duties and Responsibilities**

1.1 To work in conjunction with the Housing Allocations Team and Housing Management Workers to ensure all administrative tasks for new admissions within the Links Provision are completed and necessary documents are signed by the Service User where applicable.

1.2 To be responsible for the assessment, planning, implementation and evaluation of support services across the LINKS sites by continuously updating the Mainstay Gateway electronic system and Forum processes.

1.3 To participate in regular meetings within the LINKS ensuring individual cases are reviewed with Service Manager/Team Leader.

1.4 To offer information, advice and guidance to all Service Users and to work closely with individuals encouraging them with setting desired outcomes and to have positive aspirations.

1.5 To deliver one to one support sessions and facilitate/attend both internal and external meetings.

1.6 To record all evidence in Service User’s personal file / folders ensuring all recording systems are in place, accurate and comply with Association Policies and Procedures, giving due regard to any Housing Management or Safeguarding Risks.

2.1 To have and continuously update knowledge of Organisations that can offer a range of services to Service Users to promote independence.

2.2 To ensure effective networking relationships are established and maintained.

**3. To provide a quality service and response to Service Users at all times by continually promoting strong engagement with Service Users, encouraging them to link in with Internal and External Services to achieve their full potential.**

3.1 To work closely with specialist colleagues and colleagues from other departments and play a leading role in promoting relevant initiatives/activities and securing the attendance of caseload nominations.

3.2 To use various methods of engagement with Service Users to ensure that they sign-up to and maintain attendance at internal and external programmes of activity.

**4. To work alongside Housing Management Workers effectively and efficiently in order to manage all areas of Housing Management throughout the LINKS provision with particular emphasis on Maximising Income and Minimising Loss for the Association.**

4.1 To support the Housing Management Worker to maximise income for the Association by working with Service Users to ensure they have a personal income.

4.2 To support the Housing Management worker in the collection of all rental income including Housing Benefit, Direct Charge and current or former Rent Arrears by utilising the associated Policies and Processes.

4.3 To support the Housing Management worker in the collection of any associated debt due to an act of vandalism.

**5. General**

5.1 To participate and attend regular reviews as and when required.

5.2 To participate in any training that may be considered necessary in developing the work.

5.3 Strict adherence to confidentiality, particularly in the details of Service Users and staff is required at all times.

5.4 To work in accordance with the Association’s Safeguarding Accountability Framework at all times.

5.5 To treat everybody with respect and demonstrate commitment to Equality and Diversity.

5.6 To promote good practice through adhering to the Association’s Policy and Procedures and demonstrating the Association’s Behavioural Competencies.

5.7 To undertake any other responsibilities and duties appropriate to the level of the post, that may be required from time to time.

**Hours of Work**

4 on, 4 off Shift Pattern -07.55am - 19.00pm

**Salary**: £22,882.00-£25,791.00 per year
