TheAssociate Director of All Age NHS Continuing Health Careis accountable for leading a multi professional team to achieve high quality, safe and personalised care; ensuring the team have the key capabilities and capacity to deliver improved outcomes for our population in line with the following key standards and commitments which includes:

- The National NHS Continuing Health Care and Funded Nursing Care

Framework
- The Children and Young People’s Continuing Care National Framework,
- The postholder will ensure that the NHS CHC Team are making the best use of resources and improving equity so we make a marked difference in the inequalities in outcomes we have in our population.
- The postholder will be responsible for ensuring that there are robust and collaboratively developed and delivered local policies and protocols based on best practice and evidence to provide a responsive and timely equity of access and robust decision making.
- Key to this is the development and delivery of integrated commissioning strategies through the development and maintenance of effective system partnerships.
- The postholder will have a key role in leading efficient and effective business processes as well as demonstrating expert technical and analytical capability to deliver key quality, performance and financial indicators, working in a matrix way with other teams across the ICB.

- You will be able to provide clear leadership and management to a

multidisciplinary team across an all age service
- You will demonstrate professionalism, which we believe is essential to good care, developing and sustaining good relationships across the ICB and partner organisations in line with our ICB values
- You will be experienced in handling complex and sensitive situations and be able to make decisions in the best interest of patients and the workforce
- Possess well-developed analytical skills and demonstrate sound judgement in complex and sensitive cases
- You will be capable of securing the confidence of all parties during the

facilitation of assessments, using collaboration skills and compassion
- You will be focused on achieving evidence-based outcomes and a positive experience for individuals and their families
- You will have developed competent IT skills and be able to work well under pressure and at pace

To lead the NHS CHC agenda across the ICB, especially in relation to
planning, care pathway design and commissioning and procurement of
NHS Continuing Health Care services.
- Deputising for the Director of Nursing as appropriate.
- Being responsible for the management of quality, safety, finance, contracts and people management elements of the NHS CHC team by working in a matrix way.
- Leading a multi-professional team, line managing the senior NHS CHC

team as needed, setting ICB value-based objectives that promote effective team working, inclusive practice and maximising individual performance to be the best that they can be.
- Provide a minimum of monthly supervision of direct reports, including

ongoing appraisals for the senior team.
- Ensuring that the workforce have access to and complete all mandatory training and have a current personal development plan.
- Providing visible and effective leadership and support to the directly

managed workforce, the wider team and in the interface with other agencies and partners.
- Enabling others to understand their contribution to services provided and embracing personal development and adherence to ICB policies and procedures.
- To ensure that our professional and clinical workforce are enabled to

access regular clinical and restorative supervision, appraisals and
continuing professional development.
