Primary Care Network, working alongside our 4 GP practices in Thornton and Poulton.

This is a new and developing role. The post-holder will work in partnership with clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients.

The post-holder will work as part of the Primary Care Networks (PCNs) multi disciplinary team will assess, plan, organise and deliver clinical care in line with service and patient needs, as agreed with supervising clinicians.

**Main duties of the job**

The Nursing Associate will work with the multi professional team to deliver clinical care in line with service and patient needs, under direct and indirect supervision.

Work alongside varying clinicians to enable them to focus on the more complex clinical care.

Promote health and well-being to all patients.

Supporting with care for individuals with dementia, mental health conditions and learning disabilities.

Advise patients on general healthcare and promote self-management where appropriate.

The NMC registered Nursing Associate must be forward thinking, committed, professional and motivated, with excellent communication skills. They will have a breadth of knowledge within the field of nursing, providing holistic and person-centred care and support for patients of all ages and in a variety of settings.

**Job Summary**

Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks under the direction of a registered care professional, with a focus on promoting good health and independence.

Support with the delivery of the PCN's Care Home Team plan.

To provide high-quality holistic and person-centred care to individuals.

To provide routine care to patients as required in accordance with clinical based evidence, NICE and the NSF.

Work as part of the PCNs MDT to provide and monitor care, under direct or indirect supervision.

Improve safety and quality of care at every opportunity.

Work with the PCN MDT to ensure delivery of nursing associate duties complement existing workforce.

Work with a supervisor to take responsibility for developing own clinical competence, leadership, and reflective practice skills within the workplace, while on placements and through attending the Nursing Associate Training Programme.

Develop relationships across the MDT to support integration of the role across health and social care

Perform and record clinical observations such as blood pressure, temperature, respirations, and pulse.

After undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services.

To process pathology results as required.

To support and deliver health promotion and well-being programmes including any opportunistic delivery.

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

**Salary**: £25,147.00-£27,596.00 per year
