**Barnstaple Alliance PCN Care Co-ordinator Responsibilities**
- Carry out audits to identify improvements and maximise income for prescribing IIF (Investment and Impact fund) indicators.
- Update IT systems to include templates and protocols in order to meet the requirements of any changes in service specifications, e.g. IIF changes.
- Design efficient templates, protocols, alerts and other IT tools to support drug safety, patient care, quality record-keeping and reporting claims.
- work across Practices to share good Practice, identify information and IT needs, and implement effective systems and training. The aim is for all Practices to be best of the collaborative group by sharing what is does well.
- Establish good working relationships with people employed in Practices across the PCN to enable them to carry out their duties effectively. It is important that the skills of existing teams continue to be valued and their roles developed as agreed with the Practice.
- Work with the Clinical Pharmacists and Pharmacy technicians as well as other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN.
- Communicate with community pharmacy colleagues to assist Clinical Pharmacists and Pharmacy technicians.
- Give specific support to practices to help plan and deliver efficient vaccination campaigns such as Flu and Covid 19 using IT tools available to you.
- Take minutes of MDT meetings and disseminate; chase progress against actions identified in these meetings and ensure follow up where necessary.

**Brannam Medical Centre Prescribing Coordinator Responsibilities**

Duties will include co-ordinating and preparing repeat prescriptions for our Patients, data entry onto patient records, EPS and Patient and Pharmacy liaison. The tasks and responsibilities to be undertaken may include any or all of the items in the following lists, dependent on current and evolving practice workload and staffing levels:

- Work with our GPs, Clinical Pharmacist and Pharmacy Technician to provide a responsive repeat prescription service.
- Delivering the technical aspects of prescription preparation to ensure the safe prescribing of medicines.
- Delivering an efficient prescription service and ensuring service targets are met and errors are minimised.
- Comply with legal and professional requirements, accepted guidance on professional practice, and with safe systems of work for re-authorising repeat prescriptions.
- Run analysis reports and gather information from a variety of sources for continuous improvement, safe management of repeat prescriptions, and to co-ordinate medication reviews or changes.
- Liaise with pharmacies on the supply of prescribed drugs, alert our clinicians of any supply issues and forward information on the recommended alternatives.
- Expedite urgent prescriptions to help ensure a safe and responsive service for our Patients.
- Provide an efficient administration service as part of a Team to ensure the smooth running of the prescription support to our clinicians.
- Ensure you are keeping Patient information updated and that Patient information is always secure /kept confidential.
- Maintain accurate Patient records promptly and quickly - entering data in accordance with reporting requirements.
- Keep up to date with the Practices formulary and the National restricted drugs list.
- Bring any queries to the attention of the relevant doctor, to include under and over usage.
- Issue nursing supplies e.g. dressings, catheters if advised by a suitably trained nurse.
- On the doctors or Clinical Pharmacists instructions, inactivate drugs that have never been issued or have not been issued for a specified period of time.
- Liaise with external Pharmacy services and patients as necessary to resolve queries.
- Build good working relationships with local Pharmacies to help enable our prescription service.
- Create tasks and assist Clinicians in managing document workflow relating to prescriptions.
- Establish and maintain filing and administrative systems so that written or computerised information is easily accessible and secure.
- Ensure you have adequate stationery supplies in stock (Prescription Pads).
- To assist in the submission of claims for personally administered drugs as required.
- Ensuring the safe custody and control of all pharmaceutical products within the surgery premises, including doctors medical bags.
- Controlling the day to day purchasing of pharmacy stock items for use within the surgery.
- Escalate any problems to your line manager or practice manager that you need support with.
- Where happy and able, to provide cover for members of the Prescription team during periods of sickness and annual leave.
- Undertake training as requested to support safe and effective services for the Practice.
