**PURPOSE OF THE JOB**
To support and enable young people who are currently in care, and those who have left care and are eligible for leaving care services, to make a successful transition to adulthood through practical and planned interventions.
To provide a comprehensive and effective service, within Southwark's practice framework, assessing need, and planning and working purposefully with young people.
To support our young people, their networks and professionals, to manage risk, safeguard young people, and promote positive outcomes.
To provide a service which is relationship-based and works collaboratively with our young people, centred around our young people, and which responds in a timely manner within statutory guidelines, working collaboratively with the team.

**PRINCIPAL ACCOUNTABILITIES**
**Responsibilities**
1. Work directly and in partnership with the young person to provide a clear and accurate
assessment of the young person and their needs, in line with statutory guidance.
Undertake interviews, observations, and gather information from young people, carers,
family and friends, and other professionals as directed by the Team Manager.
2. On the basis of assessed strengths, needs and risks, devise, implement and review
pathway plans, collaboratively with the young person.
3. Manage the caseload as directed, and organise and plan work activities, prioritising tasks
and responsibilities.
4. Provide written and verbal reports, which are concise, informative and based on analysis
of complex evidence, for a range of audiences, including young people, reviews, panels
and other formal meetings, and court..
5. Work with key professionals from the department and externally to offer advice and
implement pathway plans. Contribute to professional discussion and challenge in an
appropriate manner.

**Knowledge**:
1. Knowledge of the health, education, training and work experience needs of young people and how these can be met
2. An understanding of the needs of children looked after, and care leavers, including adolescent development,
the impact of trauma, transition to independence, environmental factors, and risk and protective factors..
3. Knowledge of relevant legal and regulatory framework relating to children, young people and families, particularly leaving care regulations.
4. Knowledge and understanding of the problems and issues faced by people living in an inner city, multiracial/cultural community
5. Knowledge of roles and responsibilities of key children's agencies
6. Understanding of systemic practices, and restorative and attachment

**Experience**:
7. Experience of working directly with vulnerable children or young people, in a professional or voluntary capacity, implementing effective interventions with successful outcomes.
8. Experience of group work with young people and delivery of intervention programmes, with successful outcomes.
9. Computer literacy skills at a level to maintain case records within information management systems, and
ability to use technology to support online communication.
10.Experience of working effectively as part of a team and balancing personal workload
