The Senior Clinical Fellow will be expected to cover the Vascular and General surgical unit at middle grade (registrar) level. On call he/she will be supported by an FY1 and either an FY2 or a CT trainee and will answer to the on-call consultant. On call they will be required to cover the specialities of General Surgery, Vascular and Urology and in turn will be covered by the respective consultants in those specialities. The post-holder must respond promptly to calls from A&E and the wards (medical and surgical) to give advice and/or see and manage patients.

They would be expected to see urgent and emergency referrals from other disciplines/wards within the hospital. He/she must confer with the consultant on call and keep a record of such patients in the admission diary so that the patients could be appropriately followed up. The Senior Clinical Fellow will keep an accurate and meticulous record in patients case notes and will supervise junior medical staff to do so. All entries in the case notes should be signed (with time, date and contact number).

In addition he/she will provide clinical supervision and support to the junior medical staff. They must keep accurate record of emergency admissions and referrals and formally handover to the on call team at the end of their shift. Handover must be formal, should follow the SBAR pattern (Situation, Background, Assessment and Recommendation) Similarly they must make sure that they have understood the patients problems and needs when receiving handover from somebody else. They will supervise juniors in writing comprehensive but succinct discharge letters and summaries.

They must ensure that VTE prophylaxis assessment is undertaken and that prophylaxis is given. They must supervise, guide and teach junior members of the team in such skills as time management and team working. The Senior Clinical fellow will be expected to participate in departmental academic activities and audit of mortality and morbidity. They must demonstrate a willingness to learn and avail all opportunities to advance their clinical and operative skills.

They will be expected to attend, assist and perform operations and procedures within their ability. The Senior Clinical Fellow will undertake duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant(s), in consultation where possible with colleagues both senior and junior. Additional commitments arising under this paragraph are exceptional. In particular, the doctor will not be required to undertake work of this kind for prolonged periods or on a regular basis.

They must read and follow any protocols of medical cover laid down (either in writing or verbally) for the firm in which they are working. In case of any difficulty ensure that advice is sought from a senior colleague. NOTE: Specific duties may vary from time to time with the circumstances existing and the developing competence and interests of the members of the firm. The post-holder will be expected to cover for absent colleagues in an emergency situation (e.g.

sickness, injury, instances where an arranged locum does not arrive at short notice and similar situations). a) General Duties: i) Patients to be seen and assessed on the ward prior to surgery or admission to Intensive Care. ii) Keeping such records as may be required. These must be dated and initialled, together with any other related administrative duties, including discharge and Clinic letters.

iii) Duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant(s) in consultation where possible with colleagues both senior and junior. Additional commitments arising under this paragraph are exceptional. In particular, the doctor will not be required to undertake work of this kind for prolonged periods or on a regular basis. (iv) To read and follow any protocols and clinical guidelines laid down (either in writing or verbally) for the anaesthetic department.

(v) In case of any difficulties, either clinical or administrative, to ensure advice is sought from a senior colleague. (vi) To accurately gather data required for clinical audit and to participate in audit meetings. Keep a log book (electronic or written) of the cases anaesthetised. NOTE: Specific duties may vary from time to time with the circumstances existing and the developing competence and interests of the members of the Department.

The Vascular Surgeons run a 1:6 on-call Rota to which two surgeons based in Milton Keynes contribute. The Vascular team provide excellent surgical training, both in Vascular surgery and also some general surgery particularly hernias. Colorectal team Bedford Hospital is part of the West Anglia Cancer Network. We operated on in excess of 100 cancer cases last year at Bedford.

There are five existing colorectal surgeons providing training in open and laparoscopic resection. There is a fully integrated laparoscopic (OR1) theatre, where all
