Role Summary The PCN Care Home and Proactive Care Coordinator will serve as a key link between individual GP practices, the PCN Hub team, and local care homes. The function is essential in ensuring that all individuals receive the best possible coordinated care and access to resources. The Care Coordinator's duty is to assist colleagues and the Multi-Disciplinary Team in organising all critical activities, such as service access, MDT meetings, advice, and information, as well as ensuring that health and care planning with patients and their carers is timely, efficient, and person-centred. The job holder will work with patients, carers, and care home personnel to understand personal care requirements, which will include regular visits to care homes.

Responsibilities To play a role in the delivery of high-quality primary health care services. To complete Personalised Care Support plans with patients, carers and Care Home personnel and follow up on identified actions. Support the Care Home and Proactive Care clinical team as required. To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, coordinating and minuting multi-disciplinary team (MDT) meetings as necessary.

Following up on actions from MDT meetings. Coordinate triage process for care home referrals. Build and maintain close working relationship with Care Home and GP Practice teams. Support the PCN Manager in providing reports for submission as requested.

Be responsible for the organisation, planning and of own workload to meet set deadlines. Following PCN and practice policies and procedures as appropriate. Undertaking Weekly care home check ins. Responding to ad hoc requests from care homes.

Demonstrate effective team working inclusive of all relevant professionals. Report all accidents / incidents, and all ill health, failings in equipment and / or environment to line managers. Contribute towards audit and data collection as required. Once assessed as competent will be accountable for their own practice within their area of responsibility when identified and agreed with the line manager.

Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety. Work with the Clinical Lead to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present. Miscellaneous Work as part of the team to seek feedback, continually improve the service and contribute to business planning.

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner. Duties may vary from time to time, without changing the general character of the post or the level of responsibility.
