The post holders will be a central point of contact and navigator between identified PCNs and community teams, establishing effective communication processes across primary care and community services. Working in collaboration with the PCN core team, the post holders will also be responsible for liaising with PCN Multidisciplinary Team (MDT) leads in order to manage the coordination of attendance from community teams at MDT and Gold Standards Framework (GSF) / Palliative care meetings.

BCHC provide high quality community and specialist services within Birmingham and the West Midlands. We deliver over 100 clinical services, out in people’s homes and in over 200 healthcare settings for adults, children, people with learning disabilities, those with rehabilitation needs and also dental services. Main duties and responsibilities
- To have a comprehensive knowledge of PCN MDT meetings and priorities, ensuring that the relevant members of BCHC community teams are fully informed and involved
- To develop excellent working relationships with all stakeholders, including PCN’s MDT leads / core nucleus team and all relevant community services across BCHC
- To manage and coordinate the information flow between community nursing teams and identified PCNs, working closely with the identified PCN aligned clinician (e.g. B6 - Deputy Team Manager) or Matron to ensure effective and timely communication across the relevant teams and early resolution of any issues raised
- To support the identified PCN aligned clinician from the community team to establish and maintain effective working relationships with the PCN
- Support the PCN aligned clinician/s with preparation for the MDT meetings, collating any clinical information and patient lists as required
- Follow up non clinical actions from the MDTs, liaising with the relevant teams, both internal and external and updating records as required
- Ongoing liaison with the PCN in relation to other PCN level issues, escalating any significant issues to the PCN aligned clinician / Matron where issues these cannot be resolved by the PCN core team
- To support the development, implementation and review of standard operating procedures (SOPs) for PCN level work and processes
- To provide cover for PCN Coordinator colleagues across Community Nursing Teams within the locality during periods of absence to ensure a consistently high level of service is provided

Data management and audit
- To establish reporting mechanisms; producing and disseminating regular reports which capture activity between community teams and PCN’s and identifying any gaps/ areas for improvement, escalating as required
- Develop and undertake audits in collaboration with the PCN aligned clinician and actively participate in service/quality improvement initiatives related to this role

Training
- To provide training to new colleagues in the role, acting as a mentor throughout their induction period.
- To participate in relevant training and OD initiatives in collaboration with the PCN and community services
